Patient Education and Support

Nurse educators and nutritionists support CFHC physicians with a number of individual and group education classes. We also connect patients to a variety of social services such as case management, specialty referral and follow-up, transportation, financial counseling and housing assistance, as well as to community-based programs such as financial assistance, All Kids and FamilyCare, housing placement, WIC and mental health and substance abuse programs.
