Homeless Primary Care

Working collaboratively with Heartland Health Outreach, CFHC nurse practitioners and staff provide primary care and case management services at 15 homeless shelters on the south side of Chicago. When follow-up care is required, we refer clients to one of our three clinics or to other community programs. We serve more than 1,500 homeless clients in more than 5,500 medical, dental and case management encounters each year.
