Medical Case Management/Illinois Family Case Management program and Healthworks (DCFS)

CFHC provides medical case management and primary care services to 300 Department of Children and Family Services (DCFS) clients ages 0-21 who are wards of the state. The emphasis is on children 5 and younger and hard-to-reach, noncompliant clients from 6 to 21 years old.

We also work with the Illinois Department of Human Services to provide medical case management services for Medicaid recipients who are younger than 1 year old or who are pregnant women. Many clients on the Family Case Management program receive primary health care at CFHC, which contracts to provide case management services to 700 clients per month.
