Chronic Disease Management

As part of a health disparities collaborative focusing on diabetes and asthma, CFHC is using a federal Centers for Disease Control and Prevention model to improve patient outcomes. The program, which requires significant operational changes and improvements to deliver more effective and efficient patient care, is being expanded to all CFHC patients with diabetes.
