Chicago Family Health Center and Roseland Community Hospital form partnership to help Roseland Community

CFHC to open Health Center within Hospital to provide greater accessible to affordable, comprehensive primary outpatient health care

Now uninsured, underinsured and medically underserved residents of the Roseland Community will have a new clinic in their community which will offer greater access to affordable, comprehensive primary health care without utilizing the emergency room.

Chicago Family Health Center (CFHC) and Roseland Community Hospital (RCH) are pleased to announce the opening of the Roseland Community Health Center (RCHC), operated by Chicago Family Health Center, on the campus of the Hospital.

Opening in late spring 2006, the new health center will help to bring additional health care services into an integrated community delivery system. The RCHC will provide comprehensive primary care services from birth through geriatrics including; gynecology/ obstetrics; behavioral health; chronic disease management; medication dispensary; health education; nutrition counseling; social services; case management and outreach. The new health center will target the residents of the Roseland community who don't currently have a primary care physician. As with all of CFHC sites, services will be provided on a sliding scale basis for the uninsured.

The money for this project is coming from a collaborative grant request from CFHC and RCH to the Bureau of Primary Health Care in response to the community’s need for additional, affordable primary health care in an area. “The proposal was based on our desire to expand access to services being offered in the community while helping to alleviate a patient’s need to use the emergency room as their means of obtaining primary health care for non-emergency issues,” says Wendy R. Cox, Chief Executive Officer of Chicago Family Health Center. “This new clinic, located on the campus of RCH, will allow CFHC physicians, educators, and case managers the opportunity to treat patients for acute care, to schedule patients for primary care and provide the necessary follow-up and education for area residents.”

Since the new community health center will be located on the RCH campus, patients will be provided full service, comprehensive care including lab, x-ray and inpatient services. At full operational capacity, RCHC anticipates serving 5,400 patients per year.

Chicago Family Health Center is a community-based, non-profit health center that provides comprehensive, accessible primary healthcare to the residents of South Chicago, Roseland and surrounding communities. In 2005, CFHC served over 17,000 patients regardless of their pay.
